GENERAL REQUIREMENTS

3.	Graduates of an bachelor / undergraduate program	Valid graduation certificate/Legalized of bachelor degree S1 (For 2024 graduates can be used a graduation certificate)	jpg/pdf
4.	Have a copy of the certificate and transcript of the bachelor / undergraduate Program.	Valid transcript/ Legalized of bachelor (For 2024 graduates can be used a temporary transcript)	jpg/pdf
6.	For an Indonesian citizens with an undergraduate/bachelor degree or its equivalence from abroad, the equalization process will be follow the regulations of the Indonesian Ministry of Research, Technology and Higher Education in the Directorate of Learning.	Diploma Equalization Certificate from the Ministry of Research, Technology, and Higher Education of the Republic of Indonesia (For graduates of Indonesian citizens’ abroad)	jpg/pdf
7.	Applicants who are still working must be obtain a permit from their Direct Supervisor, who must provide proof in the form of a Letter of Approval/Learning Permit (at the time of registration must show the permit). 8.	Nursing students who have graduated from their university which accredited BAN PT/LAMPT Kes have a minimum B grade.	Certificate of Accreditation (BAN-PT/LAM-PT Kes /Ministry of Health RI) Origin S1 Study Program (Can be at graduation / most recent)	jpg/pdf
9.	Maintain a good health so that the learning process in the study program runs smoothly.	A doctor's health certificate issued by a government hospital or health agency	jpg/pdf
