GENERAL REQUIREMENTS

3.	A specialist doctor graduating from a domestic or international institution that is currently applying for a program in-line with his/her specialty. A specialist doctor graduating from an Indonesian institution must provide an adaptation certificate from an acknowledged institution by each specialist' collegium. 4.	Provide a photocopy of the Applicant's diploma for the professional specialist medicine program that has been legitimized by the Dean of the Faculty of Medicine.	A photocopy version of the Applicant's diploma	jpg / pdf

6.	For Applicants who are currently working as a civil servant (Pegawai Negeri Sipil (PNS)): Must provide a photocopy of the letter of appointment prior to and after his/her appointment as a civil servant (Surat Keputusan Pengangkatan sebagai Calon Pegawai Negeri Sipil (CPNS) and Surat Keputusan Pengangkatan sebagai Pegawai Negeri Sipil (PNS)) For Applicants who are currently part of the Police Force (TNI/POLRI): Must provide a photocopy of the first Letter of Mandate and the last Letter of Mandate (SPRIN).	For Applicants with the following working status : Civil servants (PNS): Must provide a photocopy of the letter of appointment prior to and after his/her appointment as a civil servant (Surat Keputusan Pengangkatan sebagai Calon Pegawai Negeri Sipil (CPNS) and Surat Keputusan Pengangkatan sebagai Pegawai Negeri Sipil (PNS)) Current Police Enforcement (TNI/POLRI): Must provide a photocopy of the first Letter of Mandate and the last Letter of Mandate (SPRIN).	jpg / pdf
7.	Must provide a Letter of Registration (Surat Tanda Registrasi (STR)) for a specialist doctor that is still valid (one year before the time has elapsed/prove of extension).	A valid STR Document (Minumum 6 months before its expiration).	jpg / pdf
8.	A letter of Compliance to all the rules valid within RSUD Dr. soetomo, Faculty of Medicine of UNAIR and each respective academic program.	A letter of compliance	jpg / pdf

10.	For Applicants of the learning program from the Minister of Health: Must provide a letter stating that the minister will bear the responsibility of paying the SOP and SP3 upon admittance. 11.	Provide a letter of permission/ recommendation/Mandate from the parent institution, as follows: For those with a learning program from the Minister of Health: Must provide a letter from the Regional Health Minister (Kementerian Kesehatan/Dinas Kesehatan) For those who are current members of police enforcement (TNI/POLRI): Must provide a letter from the Head of Police (Mabes TNI/KepalaKepolisianRepublik Indonesia (POLRI)).
