  1. An Indonesian citizen who graduated from a Medical Education Study Program/Faculty of Medicine accredited A/B by BAN-PT (for applicants who graduate in 2004 and later).
  2. Indonesian citizens who have graduated from College/PTN/PTS in Indonesia that have been accredited by BAN-PT (specifically for applicants who graduate graduates in 2015 and after).
Sertification Accreditations:
  1. Undergraduate Program from BAN-PT min A/B for graduates in 2004 and later.
  2. Institutions from BAN-PT for graduates in 2015 and later.

Especially for Indonesian citizens who have graduated from abroad:
  1. graduated of an accredited educational program (certificate from the accreditation board)
  2. Comply KKI requirements (adaptation program, competency test, get STR)
Especially for Indonesian citizens who have graduated from abroad:
  1. Certificate from accreditation board.
  2. Stamp duty letters follow adaptation program, competency test, and special STR management

Especially for Foreign Citizens Doctor:
  1. Obtaining a recommendation from the Indonesian Embassy in the student's home country
  2. Completing accredited study programs (certificate from the accreditation board)
  3. Have a copy of your diploma and equivalent academic transcripts available (in English)
  4. Accepted prospective students will receive Indonesian language proficiency learning for one year, as evidenced by a BIPA certificate (Indonesian Language for Foreign Speakers), as well as socio-cultural learning, with the exception of graduates from Indonesian general practitioners with an Indonesian STR.
  5. If accepted as a prospective student at UNAIR, it must have a permit/approval from the Government of the Republic of Indonesia in accordance with its authority.
Especially for Foreign Citizens Doctor:
  1. Letter of Recommendation of the Embassy of the country of origin.
  2. Certificate from accreditation board.
  3. Diploma of Academic Transcripts Bachelor (in English).
  4. One year Indonesian Language Learning skill with BIPA certificate (Indonesian for Foreign Speakers and social culture learning).
  5. Study Visa At UNAIR from Government of the Republic of Indonesia.

Do not have a physical disability or disability that would interfere with the smooth operation of studies, as evidenced by a Certificate from an authorized Government Institution/Hospital that is still valid.

Have a NAPZA Certificate (Narcotics, Psychotropics, and Other Additives) issued by RSUD Dr. Soetomo or other Government Hospitals within the last three (three) months.

Have a Graduation Certificate of Doctor's Professional and Transcript that have been legalized by the Dean of the Faculty of Medicine of origin (does not accept the conversion of the Cumulative Grade Point Average).

  1. For PPDS Candidates, Graduates of the Bachelor of Medical Education Study Program who use the Non-KBK Curriculum: have graduated as a doctor for at least 1 (one) year from the time they were declared to have passed the Indonesian Doctor Competency Examination (UKDI) by attaching a photocopy of proof of UKDI graduation and a certificate of working in Health Service Institutions (Public/Private Hospitals or Health Centers) for at least 1 (one) year.
  2. Prospective PPDS Participants must attach a Certificate of Completion of Internship (STSI) issued by the Indonesian Doctors Internship Committee (KIDI) or a Certificate of Completion of Internship (SKSI) issued by the Province.
  1. Prospective PPDS Participants who have implemented PTT must provide a photocopy of their PTT Appointment and Placement Decree as well as a Certificate of Completion of Service Period from the Ministry of Health.

12.
PPDS Participant Candidates with Civil Servants (PNS) status must attach a photocopy of the Appointment Decree as a Civil Servant Candidate (CPNS) and the Appointment Decree as a Civil Servant (PNS), while PPDS Participant Candidates with TNI/TNI Member status POLRI must attach photocopies of the First Order (Sprin) and the Last Order (Sprin).
For PPDS Participant Candidates with status:
  1. For Candidate Participants with status (PNS), must attach photocopy of SK CPNS and SK PNS.
  2. For Prospective Participants of TNI / POLRI Members, shall attach a copy of First and Last Letter of Command (Sprin).

Have a letter of approval from superiors to participate in the Specialist Medical Education Program (PPDS) (for those who are currently working).
For NON civil servants Have a Letter of Approval to follow Specialist Doctor Education Program (PPDS) from superiors (for those who are currently working).

Have a letter of Declare that states while enrolled in the Specialist Medical Education Program (PPDS) or after being designated as a specialist doctor, they are willing to be assigned anywhere in Indonesia, as known and approved by their parents/husband/wife.

19.
Prospective PPDS Participants who receive Study Assignments from the Ministry of Health/TNI/POLRI/Private Agencies must have a Certificate stating that the Ministry of Health/TNI/POLRI/Private Agency that assigns them bears the Education Costs (UKA & UKS).

20.
Have a letter of approval / recommendation / assignment from the parent agency, as follows:
  1. For prospective participants from the Ministry of Health, a letter of approval from the local Provincial Health Office is attached.
  2. For prospective PPDS participants from the TNI/POLRI who obtain a Study Assignment from the Ministry of Defense by attaching a Letter of Approval from the Headquarters of the TNI/Head of Police of the Republic of Indonesia (POLRI).

Have a City Resort Police (Polresta) Certificate of Police Record (SKCK)/Certificate of Good Conduct (SKKB) or equivalent. Prospective PPDS participants from the TNI/POLRI are required to have a legalized Certificate of Good Conduct (SKKB) from their respective units.
